Kuchisake onna (口裂け女) is the ghost of a woman who was mutilated and has come back to wreak vengeance on the world. Her name comes from the bloody gash which runs across her face, grinning from ear to ear. She appears at night to lone travelers on the road, covering her grizzly mouth with a cloth mask, a fan, or a handkerchief. She sneaks up on her victims in the dark and asks them if they think she is beautiful. If the victim answers yes, she pulls off her mask and reveals a red, blood-dripping, grotesque mouth. Then she asks in a grisly voice if they still think she is beautiful. If her victim answers no or screams in terror, she slashes him from ear to ear in an imitation of her own mutilation. If he lies and answers yes a second time, she walks away, only to follow her target home and slaughter him brutally that night.

Son nom vient de l'entaille sanglante qui défigure sa bouche, la faisant sourire d'une oreille à l'autre. Elle apparait la nuit aux voyageurs solitaires sur la route, couvrant sa bouche avec un masque ou un éventail. S'approchant furtivement de ses victimes dans l'obscurité, elle leur demande si elles la trouvent belle. Si la personne répond oui, elle retire son masque et révèle son sourire grotesque et sanglant. Puis elle lui demande d'une voix sinistre si elle la trouvent toujours belle. Si sa victime répond non ou hurle de terreur, elle l'entaille d'une oreille à l'autre en imitant sa propre mutilation. S'il ment et répond oui une deuxième fois, elle s'en va, mais suit sa cible chez elle pour la massacrer le soir même.
